Student and Resident Rotations at the Huntington VA Medical Center
Students and residents selected for rotations in a health professions training program at the Huntington VAMC will need to complete mandatory training assigned through the VA Talent Management System (TMS). All trainees will also need to complete a Student/Resident Packet including all the forms requested below and return the packet to the VA Affiliations Coordinator. Additionally, all students will be required to schedule an appointment with the Personal Identity Verification (PIV) office to get fingerprints.
TMS Mandatory Training
All trainees, including students and residents, are required to complete the “VHA Mandatory Training for Trainees” training module using the VA Talent Management System (TMS) before they can be scheduled for fingerprinting. Completion certificates for these training modules should be saved and submitted along with the rest of the student packet. Instructions can be found here: VHA Mandatory Training for Trainees
The forms below make up the student/resident packet. Packets need to be completed and returned to the VA Affiliations Coordinator (Noah Absten), in room T1-101 or another member of the Learning Resources Team at the Huntington VAMC. Some educational facilities may request that students and residents return packets directly to one of the school’s program coordinators, who will then submit the packets as a group. Please consult with your educational facility’s program coordinator prior to turning in packets directly to the VA.
***Return packets at least four weeks before your desired rotation start date***
Please complete all forms completely and accurately. Complete forms in electronic format when possible, then print and sign/date as needed. Incomplete and/or unsigned forms will be returned to the student for completion, which may delay the start of clinical rotations.

Additionally, all trainees must sign and submit an influenza vaccination form to indicate whether you have been vaccinated, provide necessary documentation if you have been vaccinated elsewhere and acknowledge that you have read, understood, and agree to comply with the requirement that in the absence of vaccination you will be expected to wear a face mask throughout the influenza season (Dec. 1 through March 31). TMS Mandatory Training must be completed and certificates returned to VAMC Huntington Learning Resources prior to scheduling to be fingerprinted. Please call the PIV Office, (304) 429-6755 ext. 4748, and make an appointment for fingerprinting at least 2 weeks prior to start date of clinical experience.
***All fingerprints must be cleared before actual start date of internship.
Please bring 2 forms of identification when coming to the PIV Office for fingerprints.
Fingerprints can be taken at any Department of Veterans Affairs Medical Center PIV Office as long as you call to schedule an appointment. Locations and phone numbers can be found here. It is very important to tell them that the fingerprint results need to be sent to the Huntington VAMC and provide them with these two numbers:SOI VA89
If you fail to provide them with these two designating numbers the results will not be sent to the Huntington VAMC. This will result in a delay or cancellation of your rotation/training.
